		Dispute resolution is required by contract between or among the parties. If so, Party A shall complete this application and attach copy of contract highlighting the dispute resolution clause. (A copy of the application will be forwarded by The Counselors to the other party or parties who will have an opportunity to file an answering statement within twenty (20) business days after notice).

The Parties hereby submit the above dispute for resolution by CRE Real Estate Dispute Resolution Program pursuant to its Protocol, rules, procedures and agreements, at a location to be designated by The Counselors subject to the agreement of the Parties. The Parties have included with this application the non-refundable administration fees required by the Fee Schedule, and the Parties agree to make further escrow deposits required by The Counselors. The Parties agree to be bound by, abide by and carry out any agreement, decision or award(s) made or rendered as a result of this dispute resolution process, and agree that a judgment may be entered upon such agreement, decision or award(s), and hereby consent to submit to the jurisdiction of any court of competent jurisdiction which may properly enter such judgment.
